When is my appointment?
You have received a confirmation email containing your appointment date, time, and provider details. Additionally, we will send reminder messages via both email and text to ensure you are informed about your upcoming appointment.
Before Your Visit: What to Know
Bringing essential items with you ahead of your visit ensures we can provide you with the best care possible. Please remember to bring:
- Insurance Card
- Valid ID (driver’s license or passport)
- List of Current Medications, including dosage
- Previous medical records or test results, if applicable
Having these items ready allows for a smoother check-in process and enables our team to thoroughly review your information before your appointment, ensuring we address your needs effectively.
We do not guarantee your provider will prescribe specific medications or fill out FMLA/disability/other paperwork. If a prescription for a controlled substance medication or FMLA/disability/other paperwork is needed, your provider may ask you to come back for additional visits to ensure that our quality and safety standards are met.
Before your appointment, you will receive communication via email and text containing a link to complete your pre-visit forms online. This includes intake forms, a medical history questionnaire, and consent forms for treatment.
Completing these forms in advance saves you time during check-in on the day of your visit and ensures your provider has essential information to prepare for your appointment.
We recommend completing your pre-visit paperwork at least 24 hours prior to your appointment date and time. Should you encounter any issues or have questions regarding your pre-visit paperwork, please don’t hesitate to contact our office.
Submitting your insurance information beforehand allows us to verify coverage, streamline billing, provide accurate cost estimates, and ensure uninterrupted access to care. Please submit your insurance details through our secure online portal or contact us for assistance.
Co-payment and fees
Please check with your insurance provider or contact our office to understand your specific copayment or fee responsibilities before your appointment. We accept credit cards, debit cards, HSA, and electronic submissions for payment.
Patients without insurance
For uninsured patients, we offer competitive self-pay rates. Please contact our office for current rates for psychiatry and therapy sessions.
Need to Reschedule or Cancel Your Appointment?
Please note that once you have booked an appointment with us it means that we have reserved time in our schedule exclusively for you. If you cancel your appointment less than 48 hours before it is scheduled to take place, you may be subject to a fee. To avoid a cancellation fee, please provide cancellation notice at least 48 hours prior to your appointment. You can cancel or reschedule an appointment by calling your practice location.
After Your Visit: Patient Portal
After our staff assists you with check-out and scheduling your next follow-up appointment, you will receive an invitation to access the Patient Portal. Here’s what you can do with the Patient Portal:
Communicate securely with our practice, ask non-urgent questions, request prescription renewals, and receive updates from your care team.
View and pay your bills online easily and securely. See your payment history and manage your finances conveniently.
Request appointments, view upcoming visits, and reschedule or cancel appointments as needed. Receive helpful reminders for your appointments.
Access and update your personal information, including contact details and insurance information. Review your health records, lab results, and medications.
Easily request prescription refills online. Track your refill requests and get notified when your medications are submitted to your pharmacy.
Contacting your Geode Office
Please review your appointment confirmation email for the phone number, email, and address of your Geode office.
Our regular hours of operation are Monday to Friday, 9:00 AM to 5:00 PM.
Additionally, we offer 24/7 support should you need to contact the office outside of these operating hours.
